IMPORTANT: How It Works

1. Use the Patient Portal to schedule your primary care visit

• Already have a Patient Portal Account? Log In
• Need to Create a Patient Portal Account? Register New Account

2. Complete your pre-visit health survey online 
If you are a new primary care patient or it’s been over a year since you’ve been seen for Primary Care at MainStreet Family Care, you will be asked to complete a Health Survey before being able to schedule your first primary care appointment. New patients will be required to submit two separate surveys.

3. Enter coupon code PCYOUTUBE when you register at the Kiosk in the clinic
On the day of your appointment, you will enter the coupon code PCYOUTUBE when you check in at the Kiosk.

4. Meet with your provider
During your appointment, the provider can prescribe one medication from our approved list.

5. Leave with your free 30-day supply
Medications are dispensed right from the clinic—no pharmacy trip needed.
