What is the Patient Portal?

The Patient Portal is key to navigating any of your primary care information or needs. The portal is where you will exclusively be able to schedule any primary care appointments, cancel or reschedule existing appointments, communicate directly with your provider, or access any patient records that are needed.

Gone are the days of calling to schedule or cancel appointments and being kept on hold. Get all of your primary care access online through our patient portal, making it easier for you to schedule an appointment, request medication refills, and message our team.

Also, don’t forget to take your Wellness Snapshot once logged in, which is a survey designed to give you a high-level look at potential health risks.

Note: This is not where you signup or log into urgent care, which is accessible on our online registration page.

Why Primary Care at Mainstreet?

The MainStreet team is dedicated to providing care for all of life’s little emergencies with our urgent care center, as well as offering constant care through our primary care!

While urgent care is helpful for unexpected illnesses and injuries, primary care keeps track of your health and ensures you are getting the care you need along the way.

Primary care allows you to establish a relationship with a healthcare provider and take better care of your health and wellness as you age. For example, you may be wondering what your risk factors are for developing heart disease. Or you may be looking for health-related information on lowering your cholesterol. We provide ongoing healthcare and monitoring for a variety of illnesses.

The MainStreet Family Care team is here to help you navigate these ongoing healthcare needs! Plus, we are open 7 days a week, making your primary care accessible even on weekends!

What If I Need To See A Specialist?

If you have an underlying issue that needs to be addressed by a specialist, our team will not only let you know that a specialist is needed, but we will also help you with referrals. Additionally, we will make sure your medical records are transferred to help you get the care you need.

Why Should I Have a Primary Care Provider (PCP)?

A primary care provider (family doctor, nurse practitioner, primary care physician, physician assistant, etc.) handles long-term health conditions (like diabetes, asthma, and high blood pressure) as well as your overall preventative health and wellness needs. We help measure your overall health through annual physicals and labwork as needed.

Consistency of care makes it easier to spot underlying problems and offers a better method of recording your health if any issues do arise. In short, having a dedicated care team allows you to stay on top of your health to better equip you to address current illnesses and issues as well as any that may arise in the future.
