What is the Patient Portal?
The Patient Portal is key to navigating any of your primary care information or needs. The portal is where you will exclusively be able to schedule any primary care appointments, cancel or reschedule existing appointments, communicate directly with your provider, or access any patient records that are needed.
Gone are the days of calling to schedule or cancel appointments and being kept on hold. Get all of your primary care access online through our patient portal, making it easier for you to schedule an appointment, request medication refills, and message our team.
Also, don’t forget to take your Wellness Snapshot once logged in, which is a survey designed to give you a high-level look at potential health risks.

What If I Need To See A Specialist?
If you have an underlying issue that needs to be addressed by a specialist, our team will not only let you know that a specialist is needed, but we will also help you with referrals. Additionally, we will make sure your medical records are transferred to help you get the care you need.
Why Should I Have a Primary Care Provider (PCP)?
A primary care provider (family doctor, nurse practitioner, primary care physician, physician assistant, etc.) handles long-term health conditions (like diabetes, asthma, and high blood pressure) as well as your overall preventative health and wellness needs. We help measure your overall health through annual physicals and labwork as needed.
Consistency of care makes it easier to spot underlying problems and offers a better method of recording your health if any issues do arise. How Do I Sign Up for Primary Care?
Signing up for primary care at MainStreet Family Care is easy! Simply click on the button below to signup for our patient portal to stay on top of your preventive care needs.
In the process, you will enter your email address and create a password that will allow you to login into the portal in the future. After creating your login information, you will enter any personal information. Once you’ve finished entering all of the necessary information, you will be taken to the Mainstreet portal dashboard. From there you can begin using the portal to schedule or manage appointments, view lab results, and access health records.
