At MainStreet Family Care, we’re committed to making primary care even more accessible. Now, when you come in for a primary care appointment, you can receive a 30-day supply of select prescriptions—completely FREE. It’s our way of supporting your health, one visit at a time.

Who Qualifies?

• Coupon must be used in conjunction with your primary care appointment.

• The medication must be on our approved list (see below).

• You may receive only one free primary care medication per visit.

IMPORTANT: How It Works

1. Use the Patient Portal to schedule your primary care visit

• Need to Create a Patient Portal Account? Register New Account

2. Complete your pre-visit health survey online 
If you are a new primary care patient or it’s been over a year since you’ve been seen for Primary Care at MainStreet Family Care, you will be asked to complete a Health Survey before being able to schedule your first primary care appointment. New patients will be required to submit two separate surveys.

3. Enter coupon code PCMED when you register at the Kiosk in the clinic
On the day of your appointment, you will enter the coupon code “PCMED” when you check in at the Kiosk.

4. Meet with your provider
During your appointment, the provider can prescribe one medication from our approved list.

5. Leave with your free 30-day supply
Medications are dispensed right from the clinic—no pharmacy trip needed.
