Before your appointment, please bring or complete the following as it applies to you:

A valid driver’s license or photo ID

A list of all medications you take, including over the counter ones

Eyeglasses or contact lenses if you use them

Hearing aids if you use them

For sleep apnea: a CPAP compliance report showing at least 90 days of use

For diabetes: your latest A1C or glucose records

For high blood pressure or heart conditions: a letter or recent test results from your specialist

A completed driver health history form (download from the DOT website or fill out at the clinic)

Any recent specialist notes if you have had major health issues

Get a good night’s sleep and stay hydrated before your appointment.

Avoid heavy, salty, or fatty meals and limit caffeine or alcohol the day before. These can affect your blood pressure or other results.

Bring your corrective lenses or hearing aids so your vision and hearing tests go smoothly.

If you have high blood pressure, diabetes, or sleep apnea, make sure these are well managed and bring proof of your treatment.

What Happens In A DOT Physical?
Our certified medical provider will complete the federally required exam. The visit includes:

A review of your health history, medications, and any chronic conditions

Measurement of height, weight, blood pressure, and pulse rate

Vision and hearing screening (you must be able to see 20/40 or better and hear a forced whisper at five feet). Please note we have a horizontal field of vision test and you will need to have at least 70 degrees field of view.

A urinalysis to check for conditions such as diabetes or kidney disease
-
A physical exam of your heart, lungs, abdomen, extremities, spine, and neurological system
If you meet DOT standards, you will receive a Medical Examiner’s Certificate (DOT medical card) valid for up to two years. Some drivers with certain conditions may receive a shorter certification period.
Medical Conditions That Could Prevent You From Getting DOT
Sometimes, you will not meet the criteria to successfully complete the evaluation. Below are some medical conditions that may affect your ability to pass the exam:
- Uncontrolled or unstable heart conditions
- Epilepsy or seizure disorders
- Menier’s Disease
- Uncontrolled diabetes
- Uncontrolled high blood pressure
- Vision loss that can’t be corrected
- Hearing loss that can’t be corrected
- Use of marijuana, even if prescribed
- Use of illegal substances
- Use of oxygen therapy
- Some respiratory conditions
- Protein in urine
The medical examiner will use their judgment to qualify you based on some of these conditions.
